Reservation for 2, Random 4 check in

This is my third time hosting. A guest made a resevation for him and his wife. He had five five star reviews. At check-in I noticed four people a woman, a teenager and two kids checking in. My capacity is only two. I quickly searched the thread on how to respond. I messaged him in the app no response. So I knocked on the door and the person said their pastor made the resevation for them. I explained the situation to them and called him. I just wanted them out and ended up refunding to get them out. How common is this? Did I get unlucky on my third resevation? How can I avoid this in the future? The only thing I could think of is his profile photo was not of his picture.
